I've arranged the form as a numbered list for simplicity. All fields must be filled in; please double-check the entries to ensure that they are correct. When you have completed entering the information, please click the button to proceed to the secure order processing site. There, you will be asked for your credit card number to complete your order.

Please be sure to read the following important notes about download passwords:

  • After your order is complete you will receive a confirmation email that is sent automatically by the credit card processing system.
  • A download password will be sent to you in a separate message within an hour of your order being completed. The password will not be in the email sent by the processing system.
  • Sometimes email gets "lost in the ether". If after an hour has passed you have not received your password, please contact me and I will send you one manually. Please remember that I am a one-man show here and I do not read my email 24 hours a day, so if there is a problem please be patient.  Thanks.
